We welcome new patients who live within our practice area to register with us. Please view the practice boundary here. If you have moved into the area from another GP practice you need to provide your previous GP plus your NHS number. This helps the NHS arrange for the transfer of your medical records from your previous practice.

Anyone in England can register with a GP surgery. It’s free to register. You do not need proof of address or immigration status, ID or an NHS number.

However, as part of our own Policies and Procedures, to ensure that we have can identify the person being registered to the correct address to avoid any fraudulent registrations which have previously occurred, we ask prospective patients to come in to the Surgery in person, show some form of photo ID, with proof of address. We never keep a copy of any of the documentation shown, we simply identify at registration with a check list.

Adults:
In order to register please fill out the GMS1 form and Patient Registration
Questionnaire (1 for each adult) and come into the Sugery with photographic ID (where available) and proof of address. We will not take a copy of any identification, this is only for verification purposes.

Children:

Same for children PLUS to bring in their red book or a copy of their immunisation records if outside of the UK. For children please fill out the GMS1 form and A Patient under 0- 15 years old form Registration Questionnaire .
