If you are due for a review of your Stroke care (usually due in/around your birthday month), and have not yet been contacted, please call the surgery reception to arrange this or alternatively you could start your review by using the link below. Our reception team will be able to arrange for a blood test followed by a telephone appointment or face to face appointment with a member of our clinical team (GP, nurse or clinical pharmacist)
